Our registered name, www.MEDLABGLOBAL.com Pty Ltd ACN 645 048 022 is trading as Medlab Global. “Dr MedLab” shall be referred herein as “Provider(s). Our providers are medically trained doctors undertaking telemedicine medical services via Dr MedLab.

 

Telemedicine service are paid in full 10 business days in advance to allow for processing.

 

Dr MedLab (Telemedicine) providers are independent of www.MedLabGlobal.com and merely using the services to communicate with you. Any information or advice received from Dr MedLab provider comes from the provider, and not from www.MedLabGlobal.com. Your interactions with the Dr MedLab provider via our services are not intended to take the place of your relationship(s) with your regular health care practitioner(s).

MedLabGlobal.com through it’s medical online service facility called Dr Medlab, Telehealth provides is a medical online service to you. MedLabGlobal.com does not make any representations or warranties about the training or skill of any providers using the services.

MedLabGlobal.com want each visitor to the site, “www.medlabglabal.com” and “you” to have a safe, pleasurable visit. We have established the following terms and conditions so that we both know what to expect from each other. By using MedLabGlobal.com you agree to be bound by the terms following. CONSENT FOR TREATMENT PERFORMED 

We are providing this information on behalf of Dr MedLab/Providers:

Telemedicine involves the use of communications to enable health care providers at sites remote from patients to provide consultative services. Providers may include primary care practitioners, specialists, and/or subspecialists. The information may be used for diagnosis, therapy, follow-up and/or education, and may include live two-way audio and video and other materials (e.g., medical records, data from medical devices).

The communications systems used will incorporate reasonable security protocols to protect the confidentiality of patient information and will include reasonable measures to safeguard the data and to ensure its integrity against intentional or unintentional corruption. At the end of each encounter, the patient will be provided an encounter summary, which may be kept for the patient’s records and may be shared with the patient’s local primary care or other provider, as appropriate.

Anticipated Benefits of Telemedicine

  • Improved access to medical care by enabling a patient to remain at his or her home or office while consulting a clinician.
  • More efficient medical evaluation and management.

Possible Risks of Telemedicine

As with any medical procedure, there are potential risks associated with the use of telemedicine. Provider believes that the likelihood of these risks materialising is very low. These risks may include, without limitation, the following:

  • Delays in medical evaluation and consultation or treatment may occur due to deficiencies or failures of the equipment.
  • Security protocols could fail, causing a breach of privacy of personal medical information.
  • Lack of access to complete medical records may result in adverse drug interactions or allergic reactions or other negative outcomes.

By accepting these Terms, you acknowledge that you understand and agree with the following:

  1. I understand that the laws that protect privacy and the confidentiality of medical information also apply to telemedicine;
  2. I understand that telemedicine may involve electronic communication of my personal medical information to medical practitioners who may be located in other areas, including out of state.
  3. I understand that I may expect the anticipated benefits from the use of telemedicine in my care, but that no results can be guaranteed or assured.
  4. With my written consent only, I understand that my healthcare information may be shared with others (including health care providers) for treatment, payment, and healthcare operations purposes.

Patient Consent to the Use of Telemedicine

I have read and understand the information provided above, and understand the risks and benefits of telemedicine, and by accepting these Terms I hereby give my informed consent to participate in a telemedicine visit under the terms described herein.

You agree to indemnify and hold MedLabGlobal.com, and our directors, officers, employees and representatives harmless from any claim or demand, including reasonable legal fees, made by any you or any third party due to or arising out of the content you submit, post to or transmit through MedLabGlobal.com. Your use of the MedLabGlobal.com your connection to the MedLabGlobal.com, your violation of the Terms of Use, or your violation of any rights of another.

YOUR ACCOUNT OBLIGATIONS

In consideration of your use of the MedLabGlobal.com, you agree to:

Provide true, accurate, current and complete information about yourself as prompted by the registration form (the “Registration Information”) and

Maintain and promptly update the Registration Information to keep it true, accurate, current and complete.

If you provide any information that is untrue, inaccurate, not current or incomplete, or we have reasonable grounds to suspect that such information is untrue, inaccurate, not current or incomplete, we may suspend or terminate your registration and refuse any and all current and future use of the Site.

You are responsible for maintaining the confidentiality of your password and registration and are fully responsible for all activities that occur under your password and registration. You agree to immediately notify us of any unauthorised use of your password or account or any other breach of security.

MedLabGlobal.com consider this registration information private. The information is kept on a secure server to protect it from outside parties. We use the information only for the limited purposes of processing your orders, for statistical purposes to improve our site and services to you, to administer our site, and to notify you of products or special offers that may be of interest to you.

You agree that authorisations to charge your payment method remains in effect until you cancel it in writing, and you agree to notify MedLabGlobal.com of any changes to your payment method. You certify that you are an authorised user of the payment method and will not dispute charges for the Services that correspond to consultation fees and services.

Account Registration

To access Providers using the Services, you must first “Register” and establish an individual user account (“Account”) by providing certain information. You agree that you will provide information truthfully, honestly, accurately and not create more than one Account, or create an Account for anyone other than yourself (with the exception of sub-accounts established for minor children of whom you are a parent or legal guardian). You agree to provide true, accurate, current, and complete information on the Account enrolment form and to keep this information current and updated as needed.

As the “Registered User and Account Holder, You” hereby certify that you are who you represent to be to MedLab Global, Dr MedLab and Providers and agree to only interact with a physician through Online Telemedicine services and through MedLab Global’s support portal. You acknowledge that your ability to access and use these services is conditional upon the truthfulness of the certifications that you make at the time of accessing a provider and that the providers you access are relying upon this certification in order to interact with you.

Your acknowledgement that “you” referred to as “I” are truthfully stating: I hereby certify that I am at least 18 years of age and am qualified under the laws of my state/country to make medical decisions on my own behalf. I acknowledge that my ability to access and use the MedLabGlobal.com, it’s systems and information is conditional upon the truthfulness of my certification of age.

I agree to abide by the responsibility agreement. I represent and warrant that I are at least 18 years of age and possess the legal right and ability, on behalf of myself or a minor child of whom I am a parent or legal guardian, to agree to these Terms of Use.

Prohibited Conduct

We can refuse to honour, not accept and cancel your order for any reason, which includes (but not limited to) pricing, product availability, product images, product descriptions, and general errors. We will do everything we can to rectify these issues as quickly as possible. LIMITED TELEMEDICINE PURCHASES 

MedLabGlobal.com reserves the right to monitor and limit the frequency of Telemedicine provider services at its sole discretion.
